The reason for the AO rating? Quite simple really. In the second chapter of the game, the cookie monster's mission is to kill big bird. He does so by raping him, slicing him up and shoving a pen through his eye. It's simple enough but upon closer inspection, it is revealed that the pen has a picture of a scantly clad woman and when the pen is clicked, what clothes the woman had on come off. That women's clothes coming off is the reason why this innocent children's game is rated AO

I heard it's adults only because Cookie Monster has intercourse with his cookies and then feeds them to homeless people.
